About the Role: 

The Community Health Worker (CHW) provides a bridge between patients at the Stephen Klein Wellness Center and their health care providers and other professionals for referrals, decision making, and overall continuity of care through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health and access to social services resources of patients. They assess any barriers to following through with healthcare needs, assisting patients in overcoming those barriers, and referring to appropriate resources as needed.

The Community Health Worker may also support with scheduling patient appointments to eliminate gaps in care and outreach to members of the community at-risk for certain health conditions.

  • Location: 2144 Cecil B. Moore Avenue, Philadelphia, PA 19121 (Stephen Klein Wellness Center)
  • Schedule: Monday through Friday
  • Pay Rate: $21.98 / hourly

Essential Duties and Responsibilities:

Specifically, the Community Health Worker will:

  • Partner with staff to increase use of services at Stephen Klein Wellness Center (SKWC)
  • Promote available services and engage patients and community members
  • Support patient registration and follow-up at SKWC
  • Identify and assist at-risk patients with Medical Assistants
  • Help with enrollment in health and social services (e.g., housing, food, transportation)
  • Navigate social service portals and accompany patients to key appointments
  • Share community resources and wellness education
  • Guide patients in setting SMART goals and developing recovery skills
  • Foster wellness, self-worth, and independence
  • Maintain and distribute updated resource materials
  • Join case reviews, team huddles, and meetings as needed
  • Assist with wellness groups, reminder calls, and front desk tasks during busy times
  • Conduct outreach to close care gaps and promote preventive care
  • Communicate effectively with diverse populations using trauma-informed care
  • Maintain professionalism, strong phone etiquette, and HIPAA compliance
  • Perform other duties as assigned

Minimum Required Experience:

  • High school graduate or GED
  • PA Driver's License
  • 1-year prior experience serving in a Community Health Worker or Certified Peer Specialist role

What Experience and Qualifications You May Bring:

  • Formal training at a CHW program
  • Experience with prenatal care
  • Certified Peer Specialist preferred
  • Member of the 19121 community
  • Experience delivering services to persons with histories of homelessness, mental illness, substance dependence, and trauma
  • Knowledge of de-escalation methods or willing to be trained in these methods
  • Bilingual (Spanish/English) preferred
  • Prior Experience with eClinical Works EMR or similar EMR

Physical Requirements

  • Ability to work for extended hours at computer workstation
