Job Summary:
The Nurse Care Manager2(NCM-2) provides a holistic, trauma informed and patient-centered approach in caring for patients seen through Project HOME's Healthcare Services Mobile Medical Team and the HUB of HOPE. Principle roles include: working closely with participants, other Project HOME (PH) stakeholders, Project HOME Healthcare Services(PHHS), and other primary care providers to provide coordinated intake assessments, goal-setting, and plans of care for patients seen by the mobile medical team and at the HUB of HOPE; collaborating with street outreach teams to coordinate care delivery for patients who are unhoused or marginally housed; assessing, triaging and responding to patients with acute, chronic, and preventive health and wellness needs; providing medical services in various settings including but not limited to clinics, private/group residences, and on the streets of Philadelphia: and providing mentorship for PHHS nurse care managers who are new to the role.

The Leasing Specialist is responsible for managing intake and housing placements for multiple permanent supportive, affordable housing programs throughout Project HOME. As the first point of contact for applicants for Project HOME, the Leasing Specialist is responsible for maintaining an in-depth knowledge of the specific residential programs and housing referral policy and procedure, creating relationships with government, city, and local referral agencies, as well as connecting with incoming applicants in a manner consistent with the mission of Project HOME. Job Summary- The Senior Program Manager (SPM) is responsible for the overall management of the Outreach Coordination Center (OCC). Through the OCC, the SPM coordinates teams of outreach workers who conduct street outreach 24-hours per day, 7-days per week. The SPM oversees daily operations, data collection and reporting and a 24- hour Homeless Outreach Hotline. The SPM also plans for weather emergencies (such as a Code Blue or a Code Red) and the SPM organizes seasonal counts of people who are unsheltered. The SPM supervises the assistant program manager of the OCC, the OCC administrative coordinator and the outreach team leads.

Job summary- The Maintenance Technician (MT) is responsible for maintaining the properties, systems, and equipment of Project HOME (PH) throughout the PH residences and facilities. The primary responsibility of the MT will be the completion of maintenance work orders as assigned. These work orders will include but not be limited to repairs, general preventative maintenance items, and other projects as deemed fit by the facilities director. Art Program Coordinator is responsible for daily creative arts classes in-person at Project HOME sites. They support Project HOME residents attending Project HOME and partner organizations' classes. And they help plan, curate, and hang at least one (1) public art exhibition.

Job Summary: The Senior Property Manager provides leadership, innovation, and vision in the management of residential facilities (multi-site supportive and affordable housing). This includes supervision of staff, financial management, and property management. In conjunction with the Residential Program Managers, the Senior Property Manager will ensure that the properties within their portfolio are properly maintained, fully occupied with high levels of rent collection while providing the residents and staff with a clean and safe environment in which to live and work. The Senior Property Manager will directly oversee Assistant Property Managers, Property Clerks, Receptionists, and internal and contracted custodial staff as applicable.
