Minimum Starting Salary: $77,136- $79,473

We are seeking a compassionate, flexible, and clinically skilled Floater Nurse Care Manager-2 to join our integrated care team and serve across multiple residential sites. This is more than a nursing job — it’s a calling to support individuals transitioning from homelessness and navigating complex medical, behavioral, and recovery challenges. As a Floater Nurse Care Manager, you’ll step into different Project HOME residences as needed, building trust, providing direct care, and guiding residents toward improved health outcomes using a holistic, trauma-informed, and person-centered approach. This role is currently assigned to the Sacred Heart Recovery Residence for four days a week and St. Elizabeth’s Recovery Residence one day a week.

Schedule: Full-Time | M-F, 8-4 pm

Key Responsibilities:

Conducts clinical assessments and procedures in compliance with OSHA standards. Works collaboratively with Program Managers, residents, and both internal and external partners to complete intake evaluations, establish health goals, and develop individualized care plans. Using a holistic, trauma-informed, and patient-centered approach, the Nurse Care Manager delivers medical and nursing support to residents at Project HOME sites. It offers a meaningful opportunity for an experienced nurse to form lasting relationships and provide high-quality care to individuals with histories of homelessness, co-occurring mental health conditions, and those in recovery from substance use.

Required Qualifications: 

  • Active Registered Nurse (RN) license in Pennsylvania
  • BSN degree
  • BLS Certification
  • Strong interpersonal, documentation, and critical thinking skills
  • Valid PA driver’s license and ability to work across multiple sites

Preferred:

  • One or more years of RN experience with adults experiencing homelessness, behavioral health conditions, or substance recovery
  • Bilingual or bicultural competence is a plus
