Nurse Practitioner

Minimum Salary: $107,116-$110,865

Project HOME is seeking a Family Nurse Practitioner to join our integrated healthcare team and help eliminate health disparities for individuals and families experiencing homelessness, housing insecurity, and systemic barriers to care. The hours are Monday-Friday, 8:00 AM-4:00 PM.

What You’ll Do:

As a key member of our community-based care team, you’ll:

  • Deliver primary care services to children and adults in both in-person and telehealth settings
  • See 9 patients per session, 8 sessions per week, with a focus on holistic and trauma-informed care
  • Collaborate with a dedicated team of medical assistants, behavioral health consultants, care managers, and peer specialists
  • Address both clinical and social determinants of health, recognizing the unique needs of marginalized populations
  • Uphold our mission and values through ethical, equitable, and evidence-based care
  • Participate in quality improvement initiatives, staff development, and clinical leadership opportunities
  • Mentor students (NP, PA, and MD) and engage in ongoing CME for professional growth

What We’re Looking For:

  • Master’s Degree and current NP license (Family Nurse Practitioner preferred)
  • CPR and DEA certification
  • Experience or strong interest in serving underserved and vulnerable populations
  • Comfort working with patients experiencing behavioral health issues, trauma, or housing insecurity
  • Strong communication, clinical, and organizational skills
  • Experience with eClinicalWorks or similar EHR systems a plus
  • Preferred: Training or experience in gynecology, obstetrics, or pediatrics

Physical Requirements:

Light lifting, standing, and walking throughout the day as needed.
