Minimum Salary: $104,503-$107,973
Job Summary: The Certified Nurse Practitioner is a key member of an integrated health care team and instrumental to its success. They provide primary medical care services and health education/screening activities to meet the healthcare needs of the Project HOME community: Project HOME residents, neighbors in the community, homeless persons & staff.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides direct patient care at Project HOME, which include:
- Primary care medical services for children and adults in consultation with and under the supervision of family physicians.
- Collaboration with Project HOME staff including medical assistants, behavioral health consultants, nurse care managers, peer recovery specialists and community health workers.
- Coordination of care for all patients
- Provider will see 9 patients per session (1/2 day) for a total of 8 sessions per week. This includes telehealth appointments in addition to in-person office visits.
Supports and demonstrates the Mission and Values of Project HOME
- Demonstrates compassion and sensitivity to all patients.
- Maintains confidentiality regarding all patients.
- Promotes awareness towards a healthy work environment and good communication among Project HOME staff.
- Takes a pro-active stance in dealing with the disenfranchised by addressing socio-economic needs in addition to clinical needs.
- Develop annual goals that align with Project HOME's mission to eliminate health disparities.
- Participate in Quality and Risk Management Committee as designated.
Leadership
- May be asked to develop programs to expand and support care delivery at Project HOME Healthcare sites
- Participate in All Staff as well Departmental meetings monthly, to include patient case presentation twice a year
- Precept students including medical, physician assistant and nurse practitioner trainees
- Maintain CME activities to provide up-to-date, patient centered, and evidence-based care
- Other duties as assigned

MINIMUM QUALIFICATIONS
- Master's Degree and current license and certification as a Nurse Practitioner
- CPR certification
- DEA License
PREFERRED QUALIFICATIONS
- Preference will be given to practitioners who have worked in an underserved area (e.g. FQHC, free clinic) and/or who have served patients who have significant behavioral health and social challenges.
- Experience in providing gynecological care, obstetrical care and pediatric care.
- Ability to establish and strengthen relationships with the community, medical partners and Project HOME staff across the organization.
- Excellent written and verbal communication skills.
- Strong computer and organizational skills.
- EHR experience, especially eClinicalWorks
PHYSICAL REQUIREMENTS
- Light lifting; standing, walking most of the day
