Nurse Practitioner Per Diem

Hourly Rate: $57.00-$59.00 per hour (based on experience)

Job Summary: Project HOME is seeking a compassionate and mission-driven Family Nurse Practitioner to join our integrated healthcare team and help eliminate health disparities for individuals and families experiencing homelessness, housing insecurity, and systemic barriers to care. This role is based at the Stephen Klein Wellness Center located art 2144 Cecil B. Moore. While our clinic hours are Monday-Friday, 8:30 AM-4:30 PM, we are open to discussing your availability to find a schedule that aligns with both your needs and our patient care goals. The Per Diem Nurse Practitioner can work anywhere between 4-30 hours, based on availability, during the various sessions Monday-Friday. Sessions are 8:30am-12:30pm and 12:30pm-4:30pm.

What You'll Do
As a key member of our community-based care team, you will:

  • Provide primary care services to children and adults through in-person and telehealth visits
  • See approximately 9 patients per session, 8 sessions per week, using a holistic and trauma-informed approach
  • Collaborate with medical assistants, behavioral health consultants, care managers, and peer specialists
  • Address both clinical needs and social determinants of health
  • Uphold ethical, equitable, and evidence-based care aligned with Project HOME's mission
  • Participate in quality improvement initiatives, staff development, and clinical leadership opportunities
  • Mentor NP, PA, and MD students and engage in ongoing continuing medical education

What We're Looking For

  • Master's degree and current NP license (Family Nurse Practitioner preferred)
  • CPR and DEA certification
  • Experience or strong interest in serving underserved and vulnerable populations
  • Comfort working with patients experiencing trauma, behavioral health concerns, or housing insecurity
  • Strong communication, clinical, and organizational skills
  • Experience with eClinicalWorks or similar EHR systems preferred
  • Training or experience in gynecology, obstetrics, or pediatrics is a plus

Physical Requirements

  • Light lifting, standing, and walking throughout the day as needed
