Job Summary: The mobile assessor connects persons who are experiencing homelessness to housing through Philadelphia's coordinated entry system, known as CEA-BHRS (Coordinated Entry and Assessment-Based Housing Referral System). This role connects vulnerable individuals and households to housing, social services and benefits. The mobile assessor will work with the street outreach teams, Office of Homeless Services (OHS) and the Department of Behavioral Health and Intellectual Disability Services (DBHIDS) to identify people who need housing assessments. |
Essential Duties and Responsibilities Housing Assessment - Conduct coordinated street outreach at least twice a week with a focus on engagement and relationship building. Identify persons and households who need housing assessments. Prioritize the By Name List (BNL) and focus lists. Respond to requests from OHS, DBHIDS and the street outreach teams to complete housing assessments.
- Complete CEA-BHRS prevention/diversion and/or housing assessments including the VISPDAT with households on the street, in shelters not funded by OHS or DBHIDS, or in locations where households experiencing homelessness gather (i.e., libraries, meal programs, engagement centers, etc.).
- Assist households with gathering necessary documentation for housing applications, which may include verifications of income, disability, and history of homelessness.
- Provide information, guidance, advocacy, and support to identified households throughout the housing assessment and match processes. Create a plan outlining next steps and barriers that may impact progress, such as substance use and or mental health disabilities.
- Assist individuals and households when scheduling interviews, completing required paperwork, or completing service referrals.
- Demonstrate a person-centered, trauma informed approach in working with households by respecting their self-determination and autonomy.
Data Collection and Reporting - Enter demographic and assessment data into HMIS; and other agency required databases.
- Provide client-level data to the Lead Agency for the Homeless Management Information System (HMIS), through direct entry into the HMIS system.
- Provide bi-weekly reports to the assistant program manager of the Outreach Coordination Center (OCC) regarding completed CEA-BHRS applications and placements.
Interagency Collaboration - Participate in required meetings to maximize collaboration and coordination with partner agencies.
- Engage with other members of the CEA-BHRS system, including the Office of Homeless Services, DBH, and external partners, to assist with evaluation, feedback, and follow-up requests.
- Participate in required training, including training related to HMIS, assessments, and other designated outreach trainings.
Minimum Qualifications - Valid driver's license
- High school diploma or Equivalent
- Three to four years of experience working with individuals with behavioral health disabilities
- Knowledge of resources available in the housing and homeless services system in Philadelphia
- Strong computer skills
- Excellent interpersonal skills and ability to resolve crisis situations appropriately.
- Excellent listening skills, communication, collaboration, and teamwork.
What Qualifications or Experience You May Bring:- College degree
- Knowledge of best practices in trauma-informed care
- Bi-lingual (Spanish and English)
- 1-2 years of experience as a case manager or resource coordinator
- Lived experience/ Peer Specialist Certificate
