Minimum Starting Salary: $104,503-$107,973
Job Summary: Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders, and health maintenance to adults and youth in an urban federally qualified health center. This includes the assessment, diagnosis, and management of mental health problems and psychiatric disorders using a state-of-the-art integrated electronic health record. The CRNP works collaboratively with on-site physicians, physician assistants, nurses, behavioral health specialists, community health workers, and ancillary staff. The NP demonstrates knowledge of and commitment to the mission of Project HOME.

- Patient Care Management
- Work collaboratively with a physician and/or mental health provider of record, (if one exists), to provide comprehensive mental health services to adults, youth, and families.
- Through patient evaluation, determine a differential diagnosis and plan for intervention.
- Adhere to standards via identified text and/or via office practice standard protocols.
- Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
- Maintain confidentiality and patient privacy.
- Document care provided in the electronic health record; meeting standards for quarterly chart reviews.
- Meet regularly with program staff to address clinical issues.
- Consultation and Referral
- Seek consultations as appropriate, act as a consultant as requested.
- Facilitate the process of ethical decision-making as appropriate.
- Perform comprehensive assessments and refer patients for further care and services within the Wellness Center, Project HOME, and/or the larger community.
- Education
- Utilize teaching-learning theory with patients and their families to influence patient outcomes.
- Identify learning needs of various populations and contribute to the development of educational programs to meet those needs.
- Provide a site for student education as requested.
- Provide on-going training to Wellness Center staff.
- Collaboration/ Process Improvement
- Identify system specific problems, which require evaluation and/or change; work with appropriate staff to resolve these issues by participating in performance improvement processes.
- Takes an active role to contribute to the improvement of office functioning.
- Perform other duties as assigned or requested.
Education and Experience Minimum Requirements:
Masters level Psychiatric-Mental Health Nurse Practitioner, national board and state certification required
3 years of experience providing clinical, systems, and administrative supervision to clinical and non-clinical staff
Experience and comfort treating a wide range of mental health issues, including severe mental disorders and substance abuse.
Experience with treating mental health needs of marginalized and vulnerable racial, ethnic, religious, LGBTQIA community required.
Experience working to integrate mental and behavioral health into primary care for marginalized/vulnerable/minority/homeless communities
Current CPR Certification.
Preferred Qualifications
Experience working with homeless individuals and/or underserved communities preferred.Experience in primary care setting preferred.Core Skills:
Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients and other behavioral health clinicians.Ability to communicate clearly in writing and verbally.Ability to function independently with regard to most clinical decision-making.Ability to handle after-hours on-call as scheduled.
