What to expect in this role:

The Resource Coordinator is responsible for providing case management services for people who are coming into the location, with a particular emphasis on people who are chronically homeless with behavioral health diagnoses. The core  case management services include engagement, building rapport, assessment, service referral, and placement.


What will you do in this role: 

  • Assessment and Referral. 
    • Build rapport with individuals who frequent the site. Learn their treatment and residential histories. Discuss what worked, what didn't work and why.
  • Referrals. 

    • Conduct assessments and refer guests to appropriate services, including shelter, permanent supportive housing, substance use and mental health treatment, and intensive case management. Complete housing applications using VISPDAT and referrals through CEA-BHRS. Assist guests with obtaining IDs and benefits as well as medical and psychiatric evaluations for placement. Promote effective strategies for engaging regular users of Hub services, including referrals to the Living Room, the Hospitality Coordinator, the benefits counselor, or the medical team as needed. Advocate for individual's needs within the larger system of services.

  • Records Management

    •  Maintain records as required for contractual and agency standards including ETO and HMIS. (Training on systems will be available.)

  • Communication

    • Participate in weekly staff meetings. Present updates on caseload and progress.

  • Review log-book at the beginning of each shift, update as necessary to maintain seamless transition between shifts.

  • Crisis Prevention. 

    • Maintain situational awareness at all times in order to prevent and respond to incidents that may arise and promote a safe environment for all.

What will you bring to this role:

  • A college degree (BA, BS or associate degree) or a high school diploma (or a high school diploma equivalent).

  • Minimum of 3-5 years of experience as a case manager, with a focus on drug and alcohol services or mental health services, or homeless services.

  • Proven ability to navigate a complex social services system and homeless provider network.

  • Experience with housing placement assistance and necessary documentation for housing applications.

  • Strong computer skills and experience with electronic case management systems.

  • Working knowledge of best practices in trauma-informed care

  • Working knowledge of the Philadelphia homeless provider network and social services system.

  • Working knowledge of the Philadelphia drug and alcohol treatment system and/or the behavioral health system
