Street Medicine Clinical Services Coordinator

Minimum Starting Salary: $42,853-$44,043

Job Summary: The PHHS Street Medicine Clinical Services Coordinator will work closely with participants, other Project HOME (PH) stakeholders, Project HOME Healthcare Services (PHHS) to provide holistic, trauma-informed, and patient-centered services to individuals experiencing homelessness/housing instability in Philadelphia. The Clinical Services Coordinator will assist in administrative duties and direct care activities that will help address the complex care needs of the patient population.

Essential Duties and Responsibilities

The Clinical Services Coordinator is a key member of the primary care team and instrumental to its success. The primary care team includes the patient, his or her family or support system, and the patient's personal clinician (attending physician, resident physician, or physician assistant, nurse). The primary care team may also include, if warranted, a pharmacist, mental health and/or substance abuse counselor, and/or peer specialist. The Clinical Services Coordinator is a multi-skilled member of the health care team who performs administrative and clinical procedures under the supervision of licensed health care providers. This is a grant funded position.

Administrative Responsibilities:

  • Engage with street medicine participants who are looking to access healthcare services
  • Complete brief intake on people requesting health care services, including obtaining signed consents to share health information between health care providers and agencies
  • Perform scheduling, insurance verification and help with insurance enrollment for participants
  • Serve as referral coordinator for the street medicine team
  • Administrative duties as required including maintaining individual health services charts
  • Ordering and tracking medical supplies and office supplies as needed
  • Participate in primary care team meetings for case review, quality and discussion of evidence-based chronic disease management, integrated care best practices, etc
  • Other Duties as assigned.

Clinical Role and Responsibilities:

  • Work effectively within a multi-disciplinary team including providers, nurses, Behavioral Health Consultants, and Community Health Workers
  • Drive outreach van as needed/transport PH street medicine staff and patients
  • Performs various clinical procedures as required including but not limited to:
    • Maintains clean and neat examination rooms, sterilization of equipment, obtaining vital signs, collect blood, tissue or other laboratory specimens, log the specimens, and prepare them for testing.
  • Provides patient counseling services and patient education consistent with directions of the supervising physician.

Education and Experience Minimum Requirements:

Minimum Experience:

  • A high school diploma or GED.
  • At least 3 years' experience working with unhoused individuals and/or people who uses substances or have mental health diagnoses.

Preferred Experience:

  • CPR and First Aid certification preferred.
  • Bilingual (Spanish-Speaking) preferred.
  • Driver's License
  • At least two years of experience as Medical Assistant

  • Proficiency in Microsoft Office, Excel, internet.
  • Valid Pa driver's license.

Physical requirements:

  • Moderate lifting, frequent, up to 25 lbs. Ability to work on their feet for the majority of the day.
