Behavioral Health Support Specialist

The Behavioral Health Support Specialist will primarily be responsible for supporting all services within the Behavioral Health Services Department, building on their experience as a Student Intern providing counseling and clinical interventions with support of Licensed Behavioral Health Consultants to patients and providing operational and patient engagement support to the MOUD Program Team and Patients in a manner that upholds the health center's mission and quality standards and results in better patient health outcomes and overall satisfaction, and stronger operational and fiscal capacity of Project HOME Healthcare services (PHHS).

Essential Duties and Responsibilities:

  • Provide clinical, operational, and care coordination support to the Behavioral Health Provider Care team (Licensed BHCs, Behavioral Health Care Coordinator) and MOUD Program

  • Contribute to overall operations of the Integrated Behavioral Health department, assisting with quality improvement projects & reporting, resource and service coordination for patients, and patient care support as needed. 

  • Under supervision of Licensed providers, conduct standardized screeners and assessments, provide clinically appropriate brief interventions, treatment support based on behavioral health assessment and integrated treatment plans in collaboration with BHCs, Integrated Medical Team, MOUD Team including facilitation/co-facilitation of MOUD Recovery groups

  • Conduct intake and re-engagement assessments with MOUD Program patients, conduct Outreach and follow up calls to MOUD patients

  • Provide operational and clinic flow support to MOUD program during patient hours, participate in weekly team meetings; monthly Behavioral Health Department Meetings as schedule allows. 

  • Identify, refer, and advocate for patients needing outside behavioral health services, and other services related to Social Determinants of Health, patient-aligned goals & needs, and barriers to wellness.

  • Facilitate community connections and resource needs of patients including completing external referrals, service applications, up to date resources/resource lists, and other patient-identified needs 

  • Participate as a member of the health center staff in the event of any other kind of emergency.

  • Complete all charting as needed and required by contracted funding sources: including assessments, progress notes, and billing.

  • Participate in planned supervision and case consultation. 

  • Fully participate in staff development opportunities offered to enhance professional growth, especially to maintain identified program performance skills 


Minimum Experience:

  • Completion of Graduate Program Field Practicum at a PHHS location

  • Master's degree in social work,counseling or related field

  • Competency in providing cognitive and behavioral interventions to individuals, families, couples, and groups with behavioral health/substance use concerns in a primary care setting

What Qualifications or Experience You May Bring:

  • Experience using trauma-informed, harm reduction, and social justice frameworks 

  • Experience working with homeless individuals and/or underserved communities
