Clinical Operations Manager (RN)

Salary: $101,000 - $113,000 based on experience

Project HOME is seeking a Clinical Operations Manager (BSN) leader who knows how to balance people, process, and purpose. This role sits at the center of our healthcare operations, supporting teams across the Stephen Klein Wellness Center, Pathways, Hub of Hope, and our Mobile and Street Medicine programs. If you are an RN who leads with both structure and compassion, this is an opportunity to make a real impact in a mission-driven environment.

Why This Role Matters
This is more than an operations role. It is an opportunity to shape how care is delivered to individuals who need it most. You will influence team performance, patient outcomes, and the overall care experience across multiple programs that serve the community every day.

Why You'll Love Working Here

Project HOME offers a competitive salary and a comprehensive benefits package, including low-cost health, dental, vision, and prescription coverage; company-paid life and disability insurance; a 401(k) with a 100% match up to 5%; paid parental leave; generous PTO including sabbatical leave; and ongoing opportunities for professional growth, leadership, and community impact-all within a deeply supportive, mission-driven team environment.

What You'll Do

  • You will lead and support nurses and medical assistants while partnering closely with the Operations Manager and Practice Managers to keep care delivery running smoothly across all sites. This includes building schedules that ensure coverage, strengthening workflows, and holding teams accountable to high standards of care and compliance.
  • You will directly supervise key clinical leaders, conduct performance evaluations, and foster a team culture grounded in collaboration and accountability. You will also oversee the Intensive Nurse Care Management Program, working alongside department directors to set goals, track outcomes, and drive results.
  • This role plays a key part in patient safety and quality. You will review clinical incidents, contribute to quality improvement efforts, and help advance Patient Centered Medical Home standards. You will also serve as a central point of coordination across medical, behavioral health, dental, pharmacy, and external partners to ensure patients receive seamless, connected care. At times, you will step into clinical support as needed, including triage, vaccine clinics, and patient care coordination.

What You Bring
You are a licensed RN in Pennsylvania with a BSN and a solid foundation in care management and team leadership. You know how to lead through others, hold standards, and still create an environment where people feel supported. Strong candidates will also bring experience working with vulnerable populations, supervising medical assistant teams, and operating in community health settings. Familiarity with trauma-informed care, person-centered recovery, and harm reduction models will set you apart.Minimum requirements include:

  • BSN and active RN license in Pennsylvania
  • 5+ years of experience as a nurse care manager
  • 3+ years of supervisory experience
  • Current BLS certification
