Program Support Specialist

As a Program Support Specialist, you will provide mobile support across Project HOME's residential sites, engaging with staff and residents to support daily programming, managing resident intake/discharge, and responding to on-site incidents as needed. This role requires flexibility, independent work, and authentic, compassionate engagement with individuals experiencing homelessness.

  • Location: Various Sites | Philadelphia, PA
  • Schedule: Full-time| Monday - Friday | 10AM-6PM or 11AM-7PM
  • Pay Rate: $23.09 per hour

What You Will Do:

  • Provide initial support, orientation, and individualized recovery planning for new residents, fostering trust and well-being as they transition from homelessness.
  • Coordinate and facilitate resources, referrals, and advocacy for residents in areas like recovery, health, employment, benefits, and supportive services, including medical and psychiatric referrals.
  • Lead group activities, house meetings, and skills-building sessions to promote community, self-efficacy, and adherence to lease and program obligations; assist residents with housing applications and discharge planning.
  • Maintain timely resident records in electronic systems, attend required meetings and trainings, collaborate with internal teams, and act as site lead when the Program Manager/Assistant Program Manager is unavailable.

SITE SPECIFIC DUTIES AND RESPONSIBILITIES AS APPLICABLE

  • Assist and support meal preparation with residents - at sites with kitchens or meal services.
  • Medication monitoring and support the organization of the medication room - at sites with medication-monitoring duties.

What You Will Bring:

  • High School Diploma/GED with 4 years' relevant experience, Associate's Degree with 2 years' experience, or Bachelor's Degree.
  • Experience supporting individuals recovering from homelessness and/or behavioral health challenges.
  • Flexible availability for evening and weekend hours
  • Valid driver's license.

Core Skills:

  • Good verbal and written communication, organizational and interpersonal skills
  • Strong computer skills and ability to learn computerized case management system.
