The Certified Peer Specialist - MAT provides integral substance use recovery supports for patients enrolled in Project HOME Health Services medication assisted treatment (MAT) services.??Supports patients in identifying needs and overcoming barriers to recovery including but not limited to integration of physical and behavioral health needs, social support, occupational and educational challenges, housing, and income stability. Because this position has potential for extended patient contact, employees will be provided with appropriate personal protective equipment.

As the first point of contact for applicants for Project HOME, the Central Intake Coordinator is responsible for maintaining an in-depth knowledge of the specific residential programs and housing referral policy and procedure, creating relationships with government, city, and local referral agencies, as well as connecting with incoming applicants in a manner consistent with the mission of Project HOME. The Coordinator facilitates the intake process, including interviews, working with referral agencies to collect required documentation, and coordinating with the Program and Property Managers to ensure that the lease signings and move-ins are occurring in a timely fashion.

Job Summary- The Mobile-Street Medicine Nurse Care Manager 1 (NCM -1) provides a holistic, trauma informed and patient-centered approach in caring for patients seen through Project HOME’s Healthcare Services Mobile Medical Team and the HUB of HOPE. Principle roles include: working closely with participants, other Project HOME (PH) stakeholders, Project HOME Healthcare Services (PHHS), and other primary care providers to provide coordinated intake assessments, goal-setting, and plans of care for patients seen by the mobile medical team and at the HUB of HOPE; collaborating with street outreach teams to coordinate care delivery for patients who are unhoused or marginally housed; assessing, triaging and responding to patients with acute, chronic, and preventive health and wellness needs; providing medical services in various settings including but not limited to clinics, private/group residences, and on the streets of Philadelphia: and providing mentorship for PHHS nurse care managers who are new to the role.

Job Summary: Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders, and health maintenance to adults and youth in an urban federally qualified health center. The Level 2 Residential Services Coordinator's role is to support residents in achieving their goals that promote wellness, recovery, and successful community living in a LGBTQ affirming environment utilizing a person-centered recovery model. The RSC 2 will assist in the development of a culture of recovery, safe and welcoming space, and peer support, in which Project HOME residents are encouraged to build and strengthen relationships with their peers and maximize their use of community resources. The RSC 2 supports residents in the retention of housing, employment, the continued development of skills and participation in activities that will enhance their ability to become self-sufficient. Schedule: Full-time -40 hours (including evenings and weekends as required). Sunday - Thursday 9-5pm.

We are looking for an energetic Part Time Assistant Teacher. In this role, you assist and provide classroom support in the after-school program with the option to work during summer camp. This role assists with daily transportation of students, which can include driving, assisting as a van aide, or walking to local schools to pick up students. Supports student learning and engagement in academic and life skills enrichment in a state-of-the-art, high-quality center.
After-school schedule is Monday- Friday, from 2:30pm until 6pm.

Job summary- The Maintenance Technician (MT) is responsible for maintaining the properties, systems, and equipment of Project HOME (PH) throughout the PH residences and facilities.
